Multiple Attendee Registration Form

Use this registration form if you are signing up multiple attendees that require a single invoice, or if you are completing a registration for someone other than yourself.

This form will be used to capture your group's information, then our staff will check the group's memberships and calculate a total for your invoice. Conference fee for current members is $350 and non-members are $390.

Please allow 7-10 days for our staff to process your request and get back to you with an invoice. Invoices can be paid online with major credit cards, or paid by check to our mailing address.

Directions to complete the form:

• The first set of contact information entered should be the organizer who will receive the invoice. 
• Once entered, click ADD GUEST to add each of your attendees to the form. 

Please note that the new ITOA Refund Policy applies to Conference registrations:

ITOA Refund Policy (effective from February 10, 2025)

The Illinois Tactical Officers Association (ITOA) refund policy for those who cancel their attendance at an ITOA paid event is as follows:

• A full refund will be issued if cancellation occurs at least 30 calendar days prior to the event. 
• If cancelled 7-29 calendar days prior to the event, a 50% refund will be issued.
• Cancellations less than 7 calendar days prior to the event will not be granted a refund.

Additionally, a registrant who does not attend the event for which they are registered will not receive a refund.
